What are targeted therapies?

Targeted therapies are advanced forms of prostate cancer treatment, as well as treatment for benign prostatic hyperplasia. They work by precisely destroying only the part of the prostate gland where the cancerous change is located — typically the most aggressive or problematic tumour focus. The healthy part of the prostate and adjacent structures remain intact.

Targeted therapies significantly reduce the risk of complications such as urinary incontinence and erectile dysfunction, which often accompany conventional radical treatment methods, including robotic surgery.

Indications for targeted therapies

Targeted therapies are intended for patients:

  • with low- or intermediate-risk prostate cancer confined to one or a limited number of foci
  • who are not eligible for radical prostatectomy (due to age or pre-existing medical conditions)
  • who decline radical surgical treatment and wish to avoid typical surgical complications (e.g. urinary incontinence, erectile dysfunction)
  • with local recurrence following radiotherapy, brachytherapy or radical prostatectomy
  • in whom cancer was incidentally detected following TURP, HoLEP, ThuLEP or similar procedures
  • who have undergone a planned HoLEP procedure as part of modern sequential HoLEP+HIFU treatmen

Focal HIFU Therapy

One of the targeted therapies is the HIFU method (High Intensity Focused Ultrasound). It uses focused ultrasound waves to precisely destroy cancerous cells within the prostate. These waves are concentrated at a single, precisely defined point, causing a localised rise in temperature and effectively destroying the tumour focus — without the need for incisions or damage to healthy tissue.

The HIFU procedure is performed using a robotic device under real-time imaging control, enabling the physician to plan and guide the treatment precisely and to protect key anatomical structures — such as the neurovascular bundles and the urethral sphincter. This radically reduces the risk of adverse effects.

Terapia TMA (Targeted Microwave Ablation)

TMA is a new step in the evolution of prostate cancer treatment. It is a method that combines the precision of fusion diagnostics with microinvasive treatment. It uses the same virtual prostate models as in fusion biopsy — meaning the physician targets exactly the same point from which the tissue sample was previously taken for diagnosis. Instead of taking a sample, an antenna emitting microwaves is introduced at that point, destroying the cancer cells in a highly precise manner.

The therapy itself takes only a few minutes, and the entire procedure is completed within one hour. This method gives patients the option of targeted removal of a prostate cancer focus without the need for radical surgery, minimises post-operative complications and preserves full quality of life.

The number of prostate cancer cases and deaths in Poland is rising. Many patients fear the consequences of surgery and the side effects of radiotherapy. Modern diagnostic and treatment methods — such as fusion biopsy, HIFU and TMA — can change this. Through targeted therapies, patients gain a safe and more comfortable alternative to radical treatment.

Patient treatment pathway at the Prostate Cancer Targeted Therapy Centre

  1. Urology consultation – Review of prior test results (e.g. prostate MRI, blood tests including PSA). You can also take advantage of a free online consultation. See details →
  2. Further diagnostics – Transperineal fusion prostate biopsy, which allows the precise location of the tumour focus to be determined. By combining MRI and ultrasound imaging, a three-dimensional reconstruction of the prostate is obtained, providing detailed information on the topography and nature of the disease and enabling advanced treatment planning. As the only centre in Poland, we routinely perform this procedure under short intravenous anaesthesia, within a 3-hour hospital stay.
  3. Eligibility assessment – A team of specialists analyses the results and assesses whether the patient qualifies for targeted therapy. The attending physician then discusses the available treatment options with the patient.
  4. Meeting with the hospital care coordinator scheduling the procedure date.
  5. Procedure – A robotic HIFU ultrasound probe is introduced transrectally, emitting waves focused on the tumour area. The entire process is monitored in real time using 3D ultrasound imaging, allowing the urologist to track the prostate boundaries, tumour location and position of adjacent anatomical structures. This enables safe preservation of the urethral sphincter and neurovascular bundles. Once the treatment plan is set, the HIFU robotic system automatically delivers ultrasound energy to the designated areas, destroying cancer cells under close physician supervision.
  6. Hospital stay – 1–2 days (same-day discharge possible in some cases).
  7. Follow-up and ongoing care – The patient remains under the continuous care of our specialists.
